Pearl Health, a leading provider of primary care physician (PCP) enablement and risk management services, announced today that it has partnered with ViCare Health, an independent physician association, to help ViCare Health physicians transition to and succeed in value-based care for Medicare patients.
Pearl Health’s technology platform combines data science with an intuitive user experience to help PCPs deliver more proactive, whole-patient care by identifying and engaging with the individuals who are most likely to benefit from proactive care interventions. The company also provides value-based financial arrangements and expert support to help physicians optimize participation and success in value-based care models.

In addition to collaborating with ViCare Health, Pearl recently announced a strategic partnership with Walgreens to enable local PCPs to deliver better-coordinated, higher-touch care to patients in the communities they serve, as well as a strategic partnership with Yuvo to ensure community-led providers have access to the same resources and capacity otherwise only accessible to large institutions.
